X-Ray Facility Registrants A. Yes, all x-ray machines must be registered with the Agency. Q. How do I register? A. Registration forms are available on our website. Completed forms may be submitted to the address on the form, or faxed to the Agency at 217-782-1328. If you have any questions or need assistance, call 217-785-9975. Q. How much does it cost? A. Registration fees vary according to the type of installation and x-ray equipment, as noted in the regulations, Section 320.10(d). The fees vary from $35 to $170 per machine per year, and are billed in January. Q. What's this fee for and why do I have to pay it? A. Effective January 1, 2000, fees for registration and inspection of x-ray equipment were combined into a revenue-neutral single annual fee, which is based on the facility and equipment type, and is billed each January. Q. How do I get inspected and how often? A. Inspection frequencies also vary by facility type, and may be annual, bi-annual, or on a 5 year cycle. Section 320.10(c) shows facility types and inspection frequencies. Q. How do I register as a diagnostic or therapy physicist? A. Forms and qualification criteria are found at http://www.illinois.gov/iema/radiation/pdf/appdistrp.pdf Q. When am I due for inspection? A. Have your registration number available and call 217-785-9975. Q. How much lead do I need for a new x-ray room? A. Lead shielding for x-ray rooms must be evaluated on a case by case basis.
